Job summary

Rise Life Services is seeking a Direct Support Professional to assist individuals in a 24-hour residential program in Riverhead, NY. You will support daily living, health services, and community integration while upholding dignity and respect for each person served.

The role requires a high school diploma, NY driver’s license, and the ability to work with individuals with developmental disabilities. Training, teamwork, and safety are emphasized within a compassionate, empowering environment.

Qualifications

  • High School Diploma required.
  • One year of experience with individuals with developmental disabilities preferred.
  • Clean New York State driver’s license required.
  • Computer skills and analytical thinking required.
  • Residential setting experience preferred; occasional lifting of up to 50 lbs.

Responsibilities

Position Summary & Responsibilities

Under the direct supervision of the Residential Manager, the Direct Support Professional must ensure the safety and well-being of the Individuals served within our 24‑hour operated OPWDD residential program. Direct Support Professionals serve as teachers and role‑models while providing support services with attentiveness to their dignity, respect, and individuality. They remain professional and communicate successfully with individuals, families, staff, management, and administration, fostering a safe, home‑like environment that is caring, empowering, and supportive to help them progress to their desired level of independence.

Responsibilities
  • Maintain professional boundaries while working with Individuals diagnosed with developmental disabilities.
  • Empower, motivate, and support Individuals in every aspect of daily living skill while promoting independence.
  • Encourage Individuals to be viable, integrated, and productive community members.
  • Build rapport with Individuals and demonstrate knowledge and understanding of developmental disabilities to better support their drive toward independence.
  • Provide OPWDD services, including but not limited to: Assertiveness/Self Advocacy training, Community Integration Services/resource, Daily Living Skills training, Health Services, Medication Management and Training, Skill Development, Socialization, Symptom management.
  • Observe, monitor and document progress or lack thereof on person‑centered service plans, reviewed monthly via PRECISION CARE; complete the Individual Monthly Summary every 30 days via electronic database.
  • Use good judgement when responding to site challenges and/or crises; maintain calm and professionalism in a fast‑paced, needs‑driven environment.
  • Document all data on the appropriate forms and ensure communication protocols are met.
  • Interpret and execute Behavior Support Plans, as applicable.
  • Safely operate a 13‑passenger vehicle to provide transportation to Individuals for appointments, recreation, grocery shopping, and emergent needs.
  • Adhere to van safety protocols, including regular gas refills, maintenance, and car washes; report concerns immediately to Residential Manager or Program Coordinator/Operations Director.
  • Work well as a team member with supervisors, co‑workers, and management; follow instructions and seek constructive feedback.
  • Maintain the residence in a safe, clean, and stress‑free manner; overnight staff is expected to deep clean the residence, while all team members keep the home clean.
  • Remain awake at all times for team members, including overnight staff.
  • Maintain compliance with all trainings and certifications throughout employment; comply with additional trainings as assigned.
  • Document monthly collateral contacts with service providers, including physicians and therapists.
  • Follow and document OPWDD fire safety regulations, such as routine fire drills and equipment checks.
  • Assist Individuals in facilitating weekly house meetings, meal preparation, recreational outings, and discussion of weekly accomplishments.
  • Maintain consistent and prompt communication with all team members, support departments, and supervisors via telephone, text, Microsoft Outlook, Microsoft Teams, and Zoom.
  • Exemplify the company’s values in everyday interactions within the residence and in the community.
Schedule Options

Full‑Time, Part‑Time, Overnight, Weekend Only, and On‑Call positions available in the following locations: Riverhead, Cutchogue, Flanders, Wading River, Shirley/Riverhead, Greenport, Ridge, Calverton, Jamesport, and Middle Island.

Qualifications
  • High School Diploma required.
  • One year of experience working with Individuals with developmental disabilities within a Residential Program (preferred).
  • A clean, valid New York State Driver’s License required.
  • Computer skills and analytical thinking required.
  • Direct Support Professional position will typically work in a residential environment with occasional work outside. Occasional lifting of 50 lbs.
