365 Health Services LLC is seeking a Direct Support Professional to work in-person serving Hereford, Pennsburg and surrounding areas. The role requires providing personal care and supporting individuals to live independently within their community.

A valid driver’s license and reliable vehicle are required. The position offers a flexible schedule with opportunities for full-time or part-time hours and a focus on person-centered care.

Qualifications

• Direct Support Professional role with experience in personal care.
• Ability to follow Individual Support Plans (ISP).
• Strong documentation and reporting skills.
• Ability to work in Hereford area; in-person availability.

Responsibilities

• Assist individuals with daily living and community integration skills.
• Support ISP goals through training and engagement.
• Provide personal care, including bathing, grooming, dressing, toileting, and hygiene.
• Assist with home-living tasks such as meal preparation, laundry, light housekeeping, and shopping.
• Monitor and report changes in condition or needs to the Executive Director or designee.
• Complete documentation and daily progress notes accurately and on time.
• Maintain safety standards, infection control practices, and proper equipment use.
• Communicate effectively with team members and maintain confidentiality.
• Attend required training and compliance programs.
• Perform other duties as assigned.

Job Type: Full-time / Part-time
Pay: $18.00 – $20.00 per hour

Overview
The Direct Support Professional (DSP) provides personal care and support services in alignment with each Individual Support Plan (ISP). DSPs help individuals maintain comfort, build independence, and achieve an everyday life within their community. A valid driver’s license and reliable vehicle are required for this role.

Key Responsibilities

• Assist individuals with daily living and community integration skills using Person-Centered Practices.
• Support ISP goals through training and engagement.
• Provide personal care, including bathing, grooming, dressing, toileting, and hygiene.
• Assist with home-living tasks such as meal preparation, laundry, light housekeeping, and shopping.
