Job summary

365 Health Services is seeking a Direct Support Professional to provide personal care and support in Downingtown, PA, in an in-person role. A valid driver’s license and reliable vehicle are required. DSPs help individuals maintain comfort, build independence, and achieve everyday life within the community.

The role includes assisting with daily living activities, documenting progress, and ensuring safety. A flexible schedule and opportunities to contribute to the community are offered.

Responsibilities

• Assist individuals with daily living and community integration skills using Person-Centered Practices.
• Support ISP goals through training and engagement.
• Provide personal care, including bathing, grooming, dressing, toileting, and hygiene.
• Assist with home-living tasks such as meal preparation, laundry, light housekeeping, and shopping.
• Monitor and report changes in condition or needs to the Executive Director or designee.
• Complete documentation and daily progress notes accurately and on time.
• Maintain safety standards, infection control practices, and proper equipment use.
• Communicate effectively with team members and maintain confidentiality.
• Attend required training and compliance programs.
• Perform other duties as assigned.

Pay: $18.00 – $20.00 per hour
