Qualifications

• 2 years of experience supporting people with developmental disabilities.
• High School diploma or better.
• Driver's License preferred.

Responsibilities

• Provides person-centered ADL, health, and safety support; assists with hygiene, grooming, meals, mobility, and other needs.
• Implements Person-Centered Plan goals; documents outcomes and tracks progress.
• Monitors and reports health, safety, behavioral, and environmental concerns; reports incidents promptly.
• Provides medication-related support within Certified Medication Technician (CMT) scope; follows procedures.
• Supports rights and self-advocacy and encourages informed decision-making.
• Engages in community inclusion through recreation, volunteer activities, and events.
• Provides safe transportation and travel training per agency requirements.
• Maintains confidentiality and documents securely; follows HIPAA and agency policy.
• Performs other duties as assigned.
