Job summary

OneWell Health Care is seeking a dedicated Caregiver/Direct Support Professional (DSP) in New Castle, PA. The role focuses on hands-on, person-centered support in home and community settings, promoting independence and well-being.

Responsibilities include daily living support, meal prep, mobility and medication assistance, and documentation. Travel to client homes and flexible scheduling are involved; prior IDD experience is preferred.

Qualifications

• High school diploma or equivalent is required.
• Prior caregiving, home health, or direct support for IDD preferred.
• Experience in community-based or residential settings is a plus.
• Driver license may be required depending on assignment.

Responsibilities

• Assist with personal care and daily living activities.
• Support meal preparation, nutrition, mobility, and medication assistance as authorized.
• Implement behavior support strategies and monitor health and safety.
• Promote independence, dignity, and community participation through structured routines.
• Provide transportation to appointments or community outings and assist with light household tasks.
