Qualifications

• Minimum age of 18 years
• Criminal background check and LEIE as required by state regulations
• Pass required controlled substance tests
• Ability to speak, read and write English
• Ability to work in stress-filled and safety-sensitive environments

Responsibilities

• Punctuality and regular attendance to ensure oversight of individuals
• Stay awake, responsive and communicate effectively with individuals
• Teach daily living skills
• Plan activities to increase quality of life
• Provide a clean, healthy, and safe environment
• Implement and follow Person Centered Support Plans and Behavioral Plans
• Supervise individuals per guidelines and support their choices
• Assist and supervise medication administration
• Complete administrative tasks within time frames
• Complete documentation required by Chrysalis and the State
• Maintain communication with the Manager regarding individuals’ needs
• Follow the Provider Code of Conduct and Abuse/Neglect policies
• Other duties as assigned by supervisor
