Job summary

You will document progress, communicate handoffs between shifts, and cooperate with DHS and regulatory inspections.

Qualifications

• Must comply with all company and regulatory policies.
• Provides person-centered care and follows individual plans.
• Must be awake and accessible in 24/7 care homes.

Responsibilities

• Administers medications as needed.
• Completes required documentation before end of shift.
• Transports individuals to appointments and assists with supervision.
• Maintains cleanliness of the home site and bedrooms.
• Coaches individuals to meet daily goals and attend activities.
