Job summary

Seven Hills Foundation seeks a Direct Support Professional II to provide daily living support in a residential setting, helping individuals live independently in their communities. You’ll follow ISP/Behavior Support Plans, assist with personal care, medications, and community activities, and mentor other staff.

Starting pay is $20.50/hour for awake hours, with location-based differentials; $17.50 for asleep hours.

Qualifications

• High School Diploma or equivalent is required.
• Active US driver’s license issued from your state of residence and must be at least 18 years old.
• Ability to work Sundays and evenings as scheduled, with mandatory Friday staff meeting if applicable.

Responsibilities

• Provide daily support and care to individuals in a residential setting.
• Help individuals build independence by teaching life skills at home and in the community.
• Follow and implement Individual Support Plans (ISP) and Behavior Support Plans.
• Assist with personal care, medication administration, and health-related needs.
• Support individuals in community activities, relationships, and social connections.
• Accompany individuals to appointments and ensure overall health and well-being.
• Maintain accurate documentation and data collection to track progress and outcomes.
• Take a leadership role by training and mentoring other staff.
