Job summary

SEVEN HILLS FOUNDATION INC is seeking a Direct Support Professional II in Hyannis, MA. You will provide daily support and care to individuals in a residential setting, helping them build independence through life skills training and community integration.

You will follow ISP and BSP guidelines, assist with personal care and medications, accompany to appointments, and document progress. Leadership duties include mentoring staff and supporting budgeting tasks in a mission-driven team environment.

Qualifications

• High school diploma or equivalent required.
• Active US driver's license from your state of residence.
• 18 years of age or older with reliable transportation and communication skills.

Responsibilities

• Provide daily support and care to individuals in residential settings.
• Teach life skills at home and in the community to promote independence.
• Follow Individual Support Plans and Behavior Support Plans.
• Assist with personal care, medication administration, and health-related needs.
• Support community activities, relationships, and social connections.
• Maintain accurate documentation to track progress and outcomes.
