Job summary

Jobtailor in Newberg, Oregon seeks compassionate direct care professionals to support residents in a 24/7 residential group home. You will help with daily living activities, health care tasks, nutrition, housekeeping, and community outings while honoring individuals’ wishes and culture.

The role requires 18 months of direct care experience or an appropriate degree, a valid driver’s license, strong communication, and the ability to lift up to 75 pounds.

Qualifications

18 months of direct care or related human services experience, or an Associate or higher degree in related human services field
Valid driver’s license and acceptable driving record
Experience following written and verbal instructions
Experience communicating clearly in writing with diverse individuals
Experience resolving conflicts professionally and promptly
Experience using computers/tablets/cell phones for documentation, reporting, and communication, including Internet, Microsoft Office, and Therap
Experience following behavioral, mental health, and/or medical supports and orders
Experience monitoring or assisting individuals with mental, emotional, behavioral, physical, or medical challenges
Ability to work days/evenings/nights/weekends/holidays/occasional overtime
Must pass criminal history and background checks and drug testing
Must be authorized to work in the United States; no visa sponsorship
Must meet ODHS SACU Driving Policy requirements
Must complete DHS Form I-9 and E-Verify employment authorization confirmation

Responsibilities

Provide direct care in a 24/7 residential group home
Support residents with behavioral supports, safety, personal care, nutrition, housekeeping, healthcare, and daily activities
Honor individuals’ wishes and culture while maintaining health and safety
Assist individuals with community access and participation in educational and recreational activities
Observe and document individuals’ activities, behavior, and progress
Administer medications and treatments under nursing and medical direction
Assist with activities of daily living and hygiene
Document and track care, answer phones, and maintain confidentiality
Prepare meals and complete housekeeping tasks
Support safe transitions back to home communities
Must be able to lift up to 75 pounds
Ability to work at nearby worksites when transferred

Support children and adults with behavioral supports, safety, personal care, nutrition, housekeeping, health care, and daily activities
Honor individuals’ wishes and culture while maintaining health and safety
Assist individuals with community access and participation in educational and recreational activities
Observe and document individuals’ activities, behavior, and progress
Administer medications and treatments under nursing and medical direction
Assist with activities of daily living and hygiene, including personal care, bathing, toileting, and feeding
Document and track all aspects of care, answer phones, and maintain confidentiality
Prepare meals and complete housekeeping tasks
Support safe transitions back to individuals’ home communities

Requirements

Eighteen months of experience providing direct individual care or training in a related human services field, or an Associate or higher-level degree in a related human services field, or an equivalent combination
Valid driver’s license and acceptable driving record
Experience following written and verbal instructions
Experience communicating clearly in writing and with diverse individuals
Experience resolving conflicts professionally and promptly
Experience using computers, tablets, and cell phones for documentation, reporting, and communication, including Internet, Microsoft Office, and Therap
Experience following behavioral, mental health, and/or medical supports and orders
Experience monitoring or assisting individuals with mental, emotional, behavioral, physical, or medical challenges
Ability to work days, evenings, nights, weekends, holidays, and occasional overtime
Ability to work at nearby worksites when transferred
Ability to lift up to 75 pounds and frequently squat, bend, and stoop
Ability to engage and maintain Oregon Intervention System-approved restraints for one hour or less
Ability to complete a three-week in-person training and obtain First Aid/CPR, Oregon Intervention System, and Medication Administration certifications
Must pass criminal history and background checks and drug testing
Must be authorized to work in the United States; the employer does not offer visa sponsorship
Must meet ODHS SACU Driving Policy requirements
Must complete DHS Form I-9 and E-Verify employment authorization confirmation
