Job summary

ELEVATE CCBHC is hiring a Direct Support Professional to oversee clients in an Intensive Residential Support Home. The role includes helping with daily living activities, meal preparation, and promoting community involvement.

The DSP may administer medications and will work across all shifts to ensure 24/7 coverage. The position requires a high school education or equivalent, a valid driver's license, and at least one year of experience with mental illness or co-occurring conditions.

Qualifications

• High school diploma or equivalent is required.
• Valid driver's license is required.
• At least 1 year experience working with individuals with mental illness or co-occurring conditions is required.
• Medication manager certification or being willing to be trained is preferred.
• Experience supporting individuals with problem behaviors is preferred.

Responsibilities

• Treat all clients with dignity, respect, and value.
• Ensure health, safety, protective supervision, and wellbeing of clients on shift.
• Immediately report any allegations of client abuse to the Program Manager.
• Complete all required trainings and orientations within the period specified by the Program Manager.
• Complete all required documentation on shift before going home, including service records and MARs.
• Support clients with implementation of personal goals and objectives.
• Provide supervision and assistance for informal supports (cooking, cleaning, laundry, meals, shopping, and personal hygiene).
• Assist with activities of daily living when absolutely needed.
• If certified to administer medications, ensure timely safe administration and MAR documentation.
• Maintain confidentiality and privacy of client information.
• Implement client behavior intervention plans when warranted.
• Report unsafe conditions to the Program Manager.
• Report changes in client condition to the Program Manager.
• Accompany clients to appointments and transports as needed.
• Other duties as requested by the Program Manager.
• Secure CPI/Mandt Training and Medication Manager training as required.

Position Summary

The direct support professional is responsible for the daily support and oversight of clients living in the Intensive Residential Support Home. The DSP provides care for individuals living with disability and/or mental health illness by providing support and assisting with daily activities in their homes. This will include working on identified goals such as meal cooking and preparation, personal cares, promoting community involvement, social skills etc. The DSP will be responsible for administering medications during the shift needed. The home operates 24 hours a day. DSP’s will be scheduled over all shifts to ensure coverage.

Shift: 2nd Shift Weekend Package --- Sunday thru Thursday 4P - 12A

Job Description

• Responsible for treating all clients with dignity, respect, and value.
• Responsible for the health, safety, protective supervision, and wellbeing of clients on your shift when working in the home.

• Responsible to complete all required documentation on your shift before going home, including, but not limited to, service documentation, data sheets, incident reports, and medication administration records.
• Responsible to provide support to clients with implementation of their personal goals and objectives.
• Responsible to provide supervision and assistance to clients with informal supports they may need help with, including but not limited to, cooking, cleaning, laundry, planning meals, shopping, and personal hygiene.
• Responsible to assist clients with activities of daily living/personal hygiene that they absolutely are unable to do for themselves.
• If certified to administer medications, you are responsible for the timely safe administration and documentation of medications in the MAR.
• Responsible to maintain the confidentiality and privacy of client information.
• Responsible to implement client behavior intervention plans when their problem behavior warrants.
• Promptly report any unsafe conditions in the home to the Program Manager.
• Reports changes in client’s condition or family situation to the Program Manager.
• Accompanies clients to scheduled appointments and transports clients or running errands with or for clients.
• Other reasonable duties as requested by the Program Manager.
• Will need to secure CPI/Mandt Training, Medication Manager training

Attendance Incentive Pay

Eligible DSP staff may receive an additional $1.50 per hour for all hours worked within a payroll period when the following criteria are met.

To qualify for the $1.50 per hour attendance incentive, the DSP must:

• Work all scheduled shifts during the payroll period
• Arrive on time for each scheduled shift
• Not leave any scheduled shift early
• Have no call-ins during the payroll period
• Have no unexcused absences

If any of the above conditions are not met during the payroll period, the attendance incentive will not apply for that payroll cycle.

Qualifications

• High school education or equivalent
• Must have a valid drivers license
• Atleast one years’ experience working with individuals with a mental illness or multi-occurring conditions
• Medication manager or certified medication aide preferred (will be required to be trained in medication manager)
• Experience supporting individuals who experience problem behaviors preferred.
