Job summary

Vinfen is seeking a dedicated case manager in the United States to provide direct case management services for persons served. You will implement service plans, train on daily living skills, and document progress in records.

You will advocate for clients, communicate with staff and providers, and assist with community outings. The role requires flexibility to cover all shifts, including evenings and weekends, in a nonprofit health and human services setting.

Qualifications

  • Typical requirements include at least one year of work experience in human services with adults with psychiatric illness or developmental disabilities.
  • Education: high school diploma or equivalent required; bachelor’s degree in counseling psychology, human services, or related field preferred or may substitute experience for training.

Responsibilities

  • Implement service plan interventions with persons served.
  • Provide direct training and support for daily living skills including symptom, medication, money management, and hygiene.
  • Maintain and update documentation in individual and program records.
  • Advocate for persons served and communicate with coworkers and service providers.
  • Escort persons served into the community using public transportation or vehicles as needed.
  • Monitor status, provide crisis intervention and counseling as required.
  • Perform housecleaning tasks and attend trainings and staff meetings.
  • Provide coverage for the program including overnights, days, evenings, weekends, and holidays.
