Job summary

The DSP will transport clients to appointments, document progress, participate in ISP planning, and work with the team to ensure safety and quality of care. CPR and PCM training are required or completed within specified windows.

Qualifications

• High school diploma or GED preferred.
• Must have valid driver’s license.
• CPR certification within 30 days of hire.
• Certified in Professional Crisis Management (PCM) within 60 days of hire.
• Ability to lift repetitively a minimum of 50 pounds.
• Good oral, written and interpersonal skills.
• Ability to maintain confidentiality.
• Ability to follow procedures, instructions, laws and policies.
• Ability to project a positive and professional image and be an effective team member.

Responsibilities

• Represent OAC professionally at community meetings, events and social functions.
• Provide day-to-day organization of activities promoting an organized, positive environment.
• Make decisions on best practices and document issues promptly.
• Coach, teach, and mentor individuals in home management and independence.
• Engage clients in ISP plans and CMS-regulated services.
• Advocate for each client’s needs and follow up when concerns arise.
• Coordinate transportation to appointments and activities; ensure required documents are prepared.
• Document client participation and progress in training objectives to teach independent living skills.
• Maintain ongoing inter-disciplinary communication and incident reporting.
