Qualifications

Responsibilities

• Participate in development of treatment plans and attend meetings.
• Document progress toward treatment plans.
• Provide interventions to help clients achieve goals and develop skills.
• Coordinate with other staff and service providers.
• Communicate progress and changes in daily notes.
• Provide ongoing support, guidance, and supervision.
• Address community integration issues with supervisor direction.
• Monitor and document medication administration.
• Ensure a clean, safe home-like environment.
• Attend core trainings and maintain required certifications.
• Provide crisis intervention and access emergency services as needed.
