Job summary

Visiting Nurse Association Community Services is seeking a Home Health Aide (HHA) / Direct Care Worker to provide compassionate personal care in clients' homes. You will deliver one-on-one in-home assistance per each client's care plan, promoting safety, comfort, and independence.

Responsibilities include helping with bathing, mobility, meals, and light housekeeping, while documenting care and communicating changes in health.

Qualifications

• Must be at least 18 years old.
• Completion of a formal certification program or ability to pass a skills test.
• Ability to read, write and follow a written Plan of Care.
• Good verbal and written communication.
• Neat appearance and personal hygiene.
• Dependability, reliability, and sound judgment.
• Able to lift up to 50 lbs.
• Maintain confidentiality and comply with HIPAA regulations.

Responsibilities

• Assist with personal care including bathing, grooming, feeding.
• Provide mobility support and transfers.
• Support physical therapy plans and exercises.
• Monitor changes in health and communicate them.
• Perform light housekeeping to maintain a safe home.
• Prepare meals and shop for groceries.

What You'll Do:

Your responsibilities may include, but are not limited to:

• Assisting with personal care: bathing, toileting, grooming, dressing, feeding, oral hygiene, nail care, shaving
• Mobility support: ambulation, transferring, positioning clients in bed, using assistive devices
• Supporting physical therapy plans, passive and active exercises under supervision
• Monitoring and communicating changes in physical or emotional health
• Performing light housekeeping: laundry, tidying bedrooms, kitchens, and bathrooms
• Preparing meals (including special diets), shopping for groceries, and household supplies
• Maintaining a clean, safe, and comfortable living environment
• Using PPE (Personal Protective Equipment) when appropriate
• Documenting care and submitting timely reports via telephony system

Who We're Looking For: Required Qualifications:

• At least 18 years of age
• Completion of a formal certification program or ability to pass a skills/competency test
• Ability to read, write, and follow a written Plan of Care
• Good verbal, written, and interpersonal communication
• Neat appearance and good personal hygiene
• Dependability, reliability, and sound judgment
• Friendly, patient, and respectful with a sincere interest in caring for others
• Ability to lift up to 50 lbs and perform physical caregiving tasks
• Maintain confidentiality and comply with HIPAA regulations

Preferred Qualifications:

• Valid driver's license, insured vehicle in good repair, and clean driving record
• Ability to travel independently to clients' homes (unless exempt)
• Must be able to work at least two weekends per month as scheduled
