Job summary

Human Service Forum is seeking Direct Care Workers to support individuals in a community residential setting, focusing on recovery and resiliency. Shifts include 1st, 2nd and 3rd, with full-time, part-time, and relief/per diem options.

The role emphasizes respect, integrity, and compassionate care in alignment with organizational guidelines. Responsibilities include daily living support, life-skills coaching, transportation to appointments, and facilitating group activities while maintaining

Qualifications

• High school diploma or GED required.
• Human services experience preferred.
• Valid driver’s license required.
• Reliable transportation during work hours.
• Auto insurance with your name listed as an insured driver on the policy.
• Ability to pass background checks and required certifications (MAP, CPR, First Aid).

Responsibilities

• Support participants with daily routines (cooking, cleaning, hygiene, etc.).
• Teach life skills and promote independence.
• Provide transportation to appointments or activities.
• Encourage self-advocacy and connect participants to community resources.
• Facilitate group activities and house meetings.
• Respond to health or behavioral concerns using supportive strategies.
• Maintain accurate documentation and uphold safety protocols.
