Job summary

Penn Home Health Care in Chester Township, PA is seeking a compassionate Direct Care Worker/Caregiver to provide one-on-one in-home support to an adult participant. This full-time, Monday–Friday role offers a stable schedule and the opportunity to help a person remain safe and independent at home.

Responsibilities include assisting with bathing, dressing, ambulation, light meals, daily living activities, and documenting care.

This is a full-time, Monday through Friday position offering a consistent schedule and the opportunity to make a meaningful difference by helping an individual remain safe and independent at home.

• Assist with activities of daily living (ADLs) and instrumental activities of daily living (IADLs)
• Promote participant safety, independence, and dignity
• Complete Electronic Visit Verification (EVV) and required documentation
• Report changes in condition, safety concerns, or incidents promptly to the agency

Qualifications

• Experience providing personal care preferred
• Direct Care Worker, Caregiver, Personal Care Aide (PCA), Home Health Aide (HHA), or Certified Nursing Assistant (CNA) experience strongly preferred
• Compassionate, dependable, and professional
• Ability to work independently while following an individualized care plan
• Reliable transportation and dependable attendance
• Excellent communication and interpersonal skills
• Must meet all Pennsylvania home care employment requirements
