Description

Provide personal care and related services in the home under the direction, instruction, and supervision of staffing and nursing personnel. Follow the plan of care to ensure safe and competent care for the participant/consumer. Assist with personal hygiene, maintaining a healthy and safe environment, and preparing nutritious meals as instructed. Support ambulation and rehabilitative processes in collaboration with therapy personnel. Encourage independence according to the care plan and promote mental alertness through engaging activities. Offer emotional and psychological support, establish trust and confidentiality with the participant/consumer and their family, and report any changes in their condition or home situation to relevant staff. Perform routine housekeeping tasks to maintain a safe environment. Attend required in-services and coordinate visits with staff to ensure proper scheduling.

Must have the ability to drive and access homes that may not be wheelchair accessible. Physical dexterity, hearing, and eyesight must be sufficient to perform assessments and care, including bending, stooping, reaching, and lifting up to 250 pounds or carrying up to 30 pounds.

Position Requirements

• High School Diploma or equivalent.
• Successful completion of one of the following:

1. Minimum training meeting HLTL, DOH, and CHHC requirements.
2. Agency-developed training program meeting PA Code 611.55 Chapter 52 criteria.
3. Nurse aide certification and training program sponsored by the PA Department of Education.

Must be free from health problems that could be injurious to the participant/consumer, self, or co-workers, with appropriate evidence provided.

Basic understanding of personal care, housekeeping, and meal preparation.

Respect for participant/consumer, including ethics and confidentiality.

Complete criminal background check.
