Direct Care Worker (Caregiver)

Description: Provide personal care and related services in the home under the direction, instruction, and supervision of staffing and nursing personnel. Follow the plan of care to ensure safe and competent care for the participant/consumer. Assist with personal hygiene, maintaining a healthy and safe environment, and preparing nutritious meals. Support ambulation and rehabilitative processes as needed, encouraging independence and mental alertness through engaging activities. Offer emotional and psychological support, establishing trust and confidentiality with the participant and family. Report any changes in physical or mental condition or home situation to appropriate staff. Perform routine housekeeping tasks to maintain a safe environment. Attend required in-services and coordinate visits with staff to ensure proper scheduling.

Position Requirements:

• High School Diploma or equivalent.
• Successful completion of one of the following: a) Minimum training meeting HLTL, DOH, and CHHC requirements; b) Agency-developed training program meeting PA Code 611.55 Chapter 52; c) Nurse aide certification and training sponsored by PA Department of Education. Certified nurse aides meet qualifications for a Personal Care Field Clinician, with additional training hours.
• Must be free from health problems that could be injurious to self, others, or the participant/consumer. Must provide appropriate health evidence.
• Basic understanding of personal care, housekeeping, and meal preparation.
• Respect for participant/consumer, including ethics and confidentiality.
• Complete criminal background check.
