A leading home healthcare provider is looking for a Direct Care Worker (Caregiver) to deliver personal care and support services to clients at home. You will work closely with nursing personnel to ensure the best care for participants while facilitating independence and promoting well-being. Strong emphasis on hygiene, meal preparation, and emotional support is required as part of the caregiver's role.

Description:
Provide personal care and related services in the home under the direction, instruction, and supervision of staffing and nursing personnel. Must follow the plan of care to provide safe and competent care to the participant/consumer. Help the participant/consumer maintain good personal hygiene and assist in maintaining a healthy and safe environment. Plan and prepare nutritious meals; assist with shopping when instructed or asked by the participant/consumer. Assist with ambulation and collaborate with therapy personnel as needed for rehabilitative processes. Encourage the participant/consumer to become as independent as possible through their care plan. Promote mental alertness through involvement in activities of interest. Provide emotional and psychological support and establish trust and confidentiality with the participant/consumer and their family. Report any changes in the participant/consumer's mental or physical condition or home situation to staffing, nursing, or office personnel. Perform routine housekeeping tasks related to a safe and comfortable environment. Attend in-services as required by regulation. Confirm weekly scheduling of visits to coordinate other necessary staff visits.
The ability to drive and access the participant's homes, which may not be routinely wheelchair accessible, is required. Hearing, eyesight, and physical dexterity must be sufficient to perform physical assessments and care. Occasionally, may need to bend, stoop, reach, and lift or carry up to 30 pounds; assist with participant weights up to 250 pounds.
Position Requirements:
