Position Overview
The Direct Care Worker (DCW) provides personal care and related services in the home under the direction, instruction, and supervision of the Clinical Director and/or appropriate supervisor. The DCW receives scheduling direction from the Client Services Coordinator and/or supervisor.
Essential Job Functions
Tasks are performed under the supervision of a registered nurse, licensed practical nurse, or appropriate supervisor. Under no circumstances may a DCW be assigned intravenous procedures, administration of medications, or any sterile or invasive procedures.
- Helps the client maintain good personal hygiene—including bathing, shaving, grooming, and dressing
- Assists in maintaining a healthy, safe environment
- Plans and prepares nutritious meals
- Completes errands as instructed by the supervisor
- Assists with transfers and ambulation with or without an assistive device
- Provides medication reminders
- Encourages the client to become as independent as possible per the nursing care plan
- Promotes mental alertness through engaging activities
- Provides simple emotional and psychological support to the client and household members
- Establishes a trusting relationship with client and family
- Maintains client/family confidentiality at all times
- Prepares a visit report and incorporates it into the clinical record weekly or as directed
- Reports changes in the client’s mental or physical condition
- Reports changes in the home situation to the company
- Carries out assignments as instructed by the supervisor, reporting pertinent changes
- Performs routine housekeeping tasks related to a safe and comfortable environment
- Participates in in‑service education per company policy and regulations
- Confirms the weekly scheduling of visits with the Supervisor/Director
Notifies the company of absences due to illness, emergency leave, vacations, bereavement, or professional meetings that will affect service.
Qualifications
- Completion of a high school diploma or equivalent preferred
- Basic math, reading, and writing skills
- Proficiency in English (speaking, reading, writing)
- Completion of a basic aide training program and/or Direct Care Worker test as required by state/federal regulations with satisfactory performance on training competency and annual re‑evaluation
- Preferred one year of experience as a personal care aide, nurse’s aide, or in another health‑care related field
- Emotional and mental maturity for establishing and maintaining good relationships with clients, families, and company personnel
- Reliable transportation to and from assignments
- Completion of all applicable health screening(s)
- A minimum of two satisfactory references and background screenings obtained prior to hire
- Valid driver’s license and acceptable MVR
Physical Requirements
- Read 12‑point or larger type
- Hear and speak in a manner understood by most people
- Stoop, bend, and lift, turn, and transfer clients weighing up to 50 pounds
- Carry bundles weighing up to 10 pounds up stairs
- Travel to prospective clients’ residences
Environment
Must be able to function in a variety of environments that may involve allergens and other conditions.
