Position Overview
The Direct Care Worker provides personal care and related services in the client’s home, working under the direction of the Clinical Director and the Client Services Coordinator. The role may be full‑time, part‑time, or PRN with flexible scheduling.
Essential Job Functions
- Assist clients with bathing, grooming, dressing, and personal hygiene.
- Maintain a safe, healthy environment for the client.
- Prepare and feed nutritious meals as directed.
- Run errands when instructed by the supervisor.
- Help clients with transfers and ambulation, using assistive devices when needed.
- Provide medication reminders.
- Encourage client independence and mental alertness through activities of interest.
- Offer emotional and psychological support and establish trusting relationships with clients and their families.
- Maintain client and family confidentiality at all times.
- Prepare a daily visit report and incorporate it into the clinical record weekly or as directed.
- Report changes in client’s mental or physical condition and changes in the home situation to the company.
- Carry out assignments as instructed by the supervisor and report any pertinent changes to care.
- Perform routine housekeeping tasks related to a safe and comfortable environment.
- Participate in in‑service education as required by company policy and state/federal regulations.
- Confirm weekly scheduling of visits with the supervisor or director.
Requirements
- High school diploma or equivalent preferred.
- Basic math, reading, and writing skills.
- Good English speaking, reading, and writing ability.
- Completion of a basic aide training program or Direct Care Worker test as required by state/federal regulations, including ongoing annual re‑evaluation.
- Preferred: One year’s experience as a personal care aide, nurse’s aide, or in another healthcare‑related field.
- Emotional and mental maturity to establish and maintain professional relationships with clients, families, and company personnel.
- Reliable transportation for all assignments.
- Completion of all required health screenings.
- At least two satisfactory references and background screenings obtained prior to hire.
- Valid driver’s license and acceptable MVR.
Physical Requirements
- Read 12‑point or larger type.
- Hear and speak in a manner understood by most people.
- Stoop, bend, and lift, turn, and transfer clients weighing up to 50 pounds.
- Carry bundles weighing up to 10 pounds up stairs.
- Travel to prospective clients’ residences as needed.
Environment
Must be able to function in a variety of environments, including exposure to allergens and other conditions.
