Job summary

Shifts are 12 hours, with training and documentation performed daily. The ideal candidate is patient, dependable, and capable of building positive relationships with residents.

Qualifications

• High school diploma or GED preferred.

• CPR/First Aid certification required or must be obtained within the required timeframe.

Responsibilities

Job Summary

We are looking for a compassionate, dependable, and responsible Direct Care Worker (DCW) to join our team! The Direct Care Worker provides hands-on care, supervision, and support to individuals living in an Adult Foster Care (AFC) home. This position is responsible for promoting the health, safety, dignity, independence, and overall well-being of each resident while following their individualized care plan. 12 hours shifts.

The ideal candidate is patient, dependable, detail-oriented, and able to build positive relationships with residents. Strong communication, teamwork, and documentation skills are essential. Previous experience in direct care, healthcare, home care, or working with individuals with disabilities is preferred.

Responsibilities

• Provide direct care and daily support to residents according to their individualized care plans
• Assist residents with activities of daily living, including bathing, grooming, dressing, toileting, feeding, and personal hygiene
• Assist residents with mobility, transfers, positioning, and transportation as needed
• Administer and assist with medications in accordance with the resident's medication administration record (MAR), physician orders, AFC policies, and applicable regulations
• Accurately document all medications administered and report medication errors, refusals, missed doses, or concerns to the appropriate supervisor
• Complete required medication training and demonstrate competency before independently administering medications
• Monitor residents for changes in physical, emotional, behavioral, or medical condition
• Promptly report changes or concerns to the supervisor and appropriate members of the resident's care team
• Complete daily documentation, progress notes, MARs, incident reports, chore logs, and other required records accurately and timely
• Prepare meals and assist residents with eating and nutritional needs
• Maintain a clean, safe, and comfortable residential environment
• Complete household duties including laundry, cleaning, organizing, and meal preparation
• Assist residents with medical appointments, community activities, errands, and other scheduled activities
• Encourage residents to participate in appropriate recreational, social, educational, and physical activities
• Provide supervision based on each resident's individual needs and care plan
• Follow resident rights, confidentiality requirements, AFC policies, safety procedures, and emergency protocols
• Maintain professional communication with supervisors, coworkers, guardians, case managers, healthcare providers, and other members of the resident's support team
• Complete all required training and maintain required certifications
• Respond appropriately to emergencies and follow established emergency procedures

Qualifications & Requirements

• High School Diploma or GED preferred
• Previous direct care, healthcare, home care, or disability-services experience preferred
• Must be dependable, responsible, compassionate, and able to work independently
• Strong verbal and written communication skills
• Ability to accurately complete required documentation
• Ability to follow individualized care plans and supervisory instructions
• Must successfully complete required medication administration training and competency requirements
• CPR and First Aid certification required or must be obtained within the required timeframe
• Required Direct Care Worker (DCW) training must be completed within the required timeframe
• Must successfully pass a required criminal history/background check prior to employment
• Must complete required fingerprinting/background screening
• Must provide a satisfactory physical examination and required TB screening/testing within the required timeframe
• Must meet all applicable AFC employment and regulatory requirements
• Reliable transportation may be required depending on the position and job duties
• Must be willing to work assigned shifts, including weekends and holidays as scheduled

Physical Requirements

The Direct Care Worker position requires the employee to be physically capable of performing essential job duties, which may include:

• Standing and walking for extended periods
• Bending, reaching, kneeling, and squatting
• Assisting residents with bathing, dressing, toileting, grooming, and other personal-care activities
• Assisting residents with transfers, positioning, mobility, and other physical needs as outlined in their care plan
• Lifting and carrying supplies and equipment as required
• Assisting with emergency situations when necessary
• Maintaining the physical ability and endurance necessary to provide safe and consistent resident care

Employees must complete any required physical examination and TB screening/testing and meet applicable health and safety requirements before beginning or continuing employment, as required.
