Job summary

Sunny Field Residential Homes LLC in Avondale, AZ is seeking a Direct Care Worker / Direct Support Professional (Caregiver) to support individuals with developmental disabilities in group home settings. This role covers daily living activities, safety, and medication administration.

The position requires a high school diploma, 1+ year experience, and certifications (Article 9, Prevention and Support, CPR/First Aid) with fingerprint clearance.

Qualifications

• High School Diploma or equivalent.
• Minimum of one year experience in DDD or Behavioral Health.
• Must obtain Fingerprint Clearance Card within 7 days of hire.
• CPR and First Aid certifications required within 90 days of hire.
• Article 9 and Prevention and Support certifications are required.

Responsibilities

• Monitor and ensure health and safety of individuals in the home.
• Administer medications as prescribed by a medical professional.
• Prepare meals (breakfast, lunch, dinner, snacks).
• Transport individuals to appointments, visits, and community events.
• Implement HAB objectives/goals with ISP team.
• Implement behavior building plans when applicable.
• Interact with individuals actively during your shift.
• Complete required documentation before leaving shift.
• Assist with monthly activity calendar and weekly menus.
• Keep the home clean and in good repair.
• Perform other duties as assigned.

Job purpose:
To plan and carry out direct care of individuals with Developmental Disabilities Adult/Children.

Key Responsibilities

• 1. Monitor and ensure the health and safety of all individuals living in the home.
• 2. Administer medications as prescribed by a medical professional.
• 3. Prepare all necessary meals (breakfast, lunch, dinner, snacks etc.).
• 4. Transport individual to appointments, visits, and activities/community events.
• 5. Implement HAB objectives/goals as agreed upon by the ISP team.
• 6. When applicable implement behavior building plans.
• 7. Actively interact with individuals in the home throughout your shift.
• 8. Complete all required Documentation before leaving shift.
• 9. Assist with preparing monthly activity calendar and weekly menus.
• 10. Ensure the home is clean and in good repair.
• 11. Other Duties as assigned.

Qualifications

• 1. Clear criminal and central registry background check (Required)
• 2. At least 18 years of age
• 3. Valid driver's license with clean driving record (If applicable)
• 4. Employee also agrees to maintain all required DDD certifications and a valid fingerprint card throughout the duration of their employment with Sunny Field Residential Homes LLC.

Education and Experience

High School Diploma and a minimum of one year of experience in the DDD or Behavioral Health Field.
