Job summary

Carries out routine case management duties as directed by more senior staff. Provides supportive contacts with clients. Carries out portions of client treatment or training plans including skills training activities. Reports to more senior staff on client behaviors; documents client progress per department policies and procedures. Assists with obtaining eligibility documentation, moving clients in and out of residential services, providing client activities and performing similar functions.

Education and/or Experience

Any combination of education and work experience equivalent to a high school diploma and two years of work experience with people with severe or persistent mental illness or two years of college.

Language Skills

Ability to read and comprehend instructions, short correspondence and memos. Ability to write simple correspondence. Ability to effectively present information in one-on-one and small group situations to customers, clients and other employees of the organization.
