Job summary

Community Bridges, Inc. (CBI) is seeking a Direct Care Specialist to join our pool of caregivers for 4 shifts a month in our Yuma, AZ operations.

You will assist with admissions, orient participants, monitor patients, and support smooth discharges within our integrated behavioral health programs. This is a part-time, on-site role with a strong emphasis on compassionate care and patient dignity.

Qualifications

• Must be a BHT or EMT certified.
• Clear 39-month MVR record and must present report at interview.
• Must have a Level 1 fingerprint clearance card.
• Must be 21 years of age.
• Behavioral health/substance abuse experience is a plus.

Responsibilities

• Assist with admission and orientation of participants.
• Ongoing monitoring of participants in the facility.
• Assist with discharges and transition planning.

Skills

BHT or EMT certification

Clear MVR on file (39 months)

Level 1 fingerprint clearance

Must be 21+ years old

Behavioral health experience a plus

Direct Care Specialist - Pool (4 Shifts a Month)

Job Summary

Direct Care Specialist is responsible for assisting with the admission and orientation of participants of the program. Direct Care Specialist is also responsible for the ongoing monitoring of each patient in the voluntary and involuntary facility as well as discharges.

Skills/Requirements

• Must be a BHT or have EMT certification.
