Job summary

Waystone Health and Human Services is seeking a Direct Support Professional in Haverhill, MA to assist four adults with ADLs in a group home setting. The role includes medication administration, transporting clients in wheelchair-accessible vans, and maintaining daily logs and ISP progress toward individual goals.

Qualifications

• High school diploma or GED; MAP certification training offered within 6 months.
• Strong reading, writing, and comprehension of English to follow care plans and document services.
• CPR/First Aid certification or training encouraged; MAP training provided by Waystone.

Responsibilities

• Assist individuals with daily living activities, including hygiene, feeding, dressing, and mobility.
• Provide transportation using wheelchair-accessible vans to appointments and activities.
• Complete ISP plans, logs, and notes; track behavioral data.
• Administer medications as required and maintain accurate documentation; provide companionship and redirect behaviors.
