Job summary

CareSpot LLC in Phoenix, AZ is seeking a DTA-Direct Care Worker to provide habilitative training, supervision, and personal care for adults with developmental disabilities at a DTA location.

You will help members acquire or improve social, communication, and living skills, using person-centered plans and with staff present. Requires HCBS eligibility, fingerprint clearance, CPR/First Aid, and a valid AZ driver’s license.

Qualifications

• Minimum qualifications include 18 years or older, professional references, and ability to perform DSP duties.
• Fingerprint Clearance Card and compliance with Arizona fingerprint rules.
• Current CPR and First Aid certifications from certified instructors.
• Orientation and training curriculum covering health/safety, incidents, abuse/neglect, medication procedures, and emergencies.
• HCBS certification eligibility as required by OLCR for DTA/HCBS service provision.
• Valid Arizona driver’s license (if transporting members) and acceptable driving record.

Responsibilities

• Provide habilitative training, supervision, and personal care to adults with developmental disabilities.
• Facilitate community integration and access to resources.
• Document attendance, services, progress, and incidents accurately.
• Communicate effectively with members, guardians, coordinators, clinicians, and leadership.
• Adhere to health/safety policies and complete required reports within 24 hours.
