Job summary

LIFE PACE is seeking a compassionate Dietitian to support the nutritional health and well-being of older adults in our Program of All-Inclusive Care for the Elderly. The Dietitian will assess participants’ needs, develop interventions, provide education, and collaborate with an interdisciplinary team for person-centered care.

Must hold a Bachelor's in Dietetics, Oklahoma Dietitian license, ADA registration, and a current Driver's License; 3+ years in clinical dietetics preferred.

Qualifications

• Bachelor's Degree in Dietetics or equivalent is required.
• Current Dietitian license in the State of Oklahoma is required.
• ADA Registration current and active.
• Driver's License current and valid.

Responsibilities

• Complete initial nutritional assessments for LIFE PACE participants.
• Develop and maintain individualized nutrition care plans and documentation.
• Provide individual and group nutrition education and cooking demonstrations.

Seeking a Dietician to Join Our Team!

LIFE PACE is seeking a compassionate and collaborative Dietitian to support the nutritional health and overall well-being of older adults in our Program of All-Inclusive Care for the Elderly. This role assesses participants’ nutritional needs, develops appropriate interventions, provides education, and works closely with our interdisciplinary team to deliver coordinated, person-centered care.

What You’ll Do:

• Complete initial nutritional assessments for all LIFE PACE participants and six-month assessments for participants requiring nutritional intervention.
• Participate in interdisciplinary team meetings, care planning, falls meetings, quality meetings, staff meetings, in-services, committees, and special projects.
• Develop and maintain individualized nutrition care plans, diet orders, and supporting documentation.
• Monitor participant weight changes and provide timely interventions when significant changes occur.
• Recommend and coordinate nutritional interventions, including supplements, home-delivered or frozen meals, food pantry resources, and nutrition education.
• Present nutrition-related service requests to the interdisciplinary team as needed.
• Provide individual and group nutrition education, including classes and cooking demonstrations for LIFE PACE and Adult Day Health participants.
• Order nutritional supplements and review related invoices for payment.
• Coordinate home-delivered meal services and maintain positive relationships with vendors.
• Document nutrition interventions, meal deliveries, education, and participant outcomes accurately and promptly.
• Complete quality-assurance activities for the nutrition discipline.
• Contribute nutrition information to organizational newsletters or publications as requested.
• Deliver food or nutritional items to participants when needed.
• Follow all organizational policies and requirements related to confidentiality, HIPAA, infection control, personal protective equipment, blood-borne pathogens, hazardous materials, and fire safety.
• Perform other duties and participate in committees or task groups as assigned.

Requirements

• Bachelor's Degree in Dietetics, Nutrition or equivalent.
• Hold and maintains a current Dietician licensed in the State of Oklahoma.
• Hold and maintains a current A.D.A. registration.
• Hold and maintains a current Driver's License.
• Upon hire, will obtain and maintain current certifications in first aid and CPR.

CERTIFICATION, LICENSES, REGISTRATIONS:

• Bachelor's Degree in Dietetics, Nutrition or equivalent.
• Hold and maintains a current Dietician licensed in the State of Oklahoma.
• Hold and maintains a current A.D.A. registration.
• Hold and maintains a current Driver's License.
• Upon hire, will obtain and maintain current certifications in first aid and CPR.

EDUCATION AND EXPERIENCE:

• 3+ years’ experience as a clinical dietitian in an outpatient and or community-based setting preferred.
• Experience in working with a diverse, elderly population, preferable in a community-based setting or geriatric program

KNOWLEDGE, SKILLS AND ABILITIES:

• Must have excellent written, oral and interpersonal skills including phone etiquette.
• Must have flexibility and personal integrity.
• Must demonstrate on an ongoing basis the ability to develop and maintain good working relationships with co-workers and department heads while retaining strict confidentiality in all aspects of the position.
• Must have advanced computer skills including Microsoft Excel, Outlook and Word.
• Must have experience utilizing office equipment including phone systems, photocopiers and fax machines.
• Must be detail and accuracy oriented and complete tasks in a timely manner.
• Must attend continuing education courses as needed
• Must be a team player with leadership ability.

PHYSICAL REQUIREMENTS:

Requires constant hand grasp and finger dexterity; frequent sitting, standing, walking and repetitive leg and arm movements, occasional bending, reaching forward and overhead, squatting and kneeling. Ability to communicate verbally, with an excellent comprehension of the English language. Has access to and ability to drive one’s own vehicle.
