Job summary

Nutrition that Works, LLC is seeking a consultant dietitian to serve our long-term care community in Charlotte Hall, MD. The role is 16 hours per week with flexible scheduling and competitive pay.

Responsibilities include clinical nutrition assessments, Nutrition Care Process documentation in EMR, MDS - Section K, and collaboration with the care team to optimize wound care and weight management.

Qualifications

• Credentials: RD with CDR credential and state licensure.
• Experience performing clinical nutrition assessments and documenting with the Nutrition Care Process.
• Strong communication and client-centered care in a team setting.

Responsibilities

• Conduct clinical nutrition assessments.
• Document using the Nutrition Care Process in the EMR system.
• Collaborate with the interdisciplinary care plan team.
• Complete the MDS - Section K.
• Calculate TF.
• Make recommendations for wounds and significant weight changes.
