Job summary

Nutrition that Works, LLC is seeking a consultant dietitian to support our long-term care community in Baltimore, MD, on a 16-40 hours/week basis. The role focuses on clinical nutrition assessments and care planning within an interdisciplinary team, using the Nutrition Care Process in the EMR.

You will also complete the MDS - Section K and contribute to wound and weight-change recommendations. Ideal candidates are credentialed by the CDR, hold a dietetic licensure, and bring strong communication

Qualifications

• CDR credential required.
• Dietitian licensure required.
• Positive attitude and client-centered approach.
• Strong communication skills.

Responsibilities

• Complete clinical nutrition assessments.
• Document care using the Nutrition Care Process in the EMR.
• Collaborate with the interdisciplinary care plan team.
• Complete the MDS - Section K.
• Calculate TF as needed.
• Make recommendations for wounds and weight changes.
