Responsibilities

  • Conduct comprehensive nutritional assessments on admission and as needed.
  • Develop individualized nutrition care plans based on medical conditions and personal preferences.
  • Monitor weight, dietary intake, and nutritional status; adjust plans as needed.
  • Collaborate with the interdisciplinary team to meet regulatory guidelines.
  • Plan menus to meet nutritional needs and therapeutic diets.
  • Educate staff on food safety and nutrition best practices.
  • Document assessments and progress in residents' medical charts.
  • Participate in care plan meetings with the team.

Job description

We are seeking a dedicatedRegistered Dietitian (RD)to join our team and provide high-quality nutritional care for our residents. The RD will assess nutritional needs, develop individualized meal and care plans and collaborate with the Interdisciplinary team to ensure compliance with all federal, state, and facility regulations.

Key Responsibilities:
Resident Nutrition Care & Assessment
  • Conduct comprehensive nutritional assessments for residents upon admission, quarterly, and as needed.

  • Develop and implement individualized nutrition care plans based on medical conditions, dietary restrictions, and personal preferences.

  • Monitor residents' weight, dietary intake, and overall nutritional status, making recommendations as necessary.

  • Work with the interdisciplinary team to support residents with conditions such as diabetes, dysphagia, renal disease, and malnutrition.

Menu Planning & Compliance
  • Ensure menus meet residents' nutritional needs and comply with regulatory guidelines.

  • Collaborate with dietary staff to modify meals for therapeutic diets and special dietary needs when needed.

  • Educate staff on food safety, portion control, and nutrition-related best practices.

Regulatory Compliance & Documentation
  • Maintain accurate records of nutritional assessments, interventions, and progress notes in residents' medical charts.

  • Ensure compliance with federal, state and local regulations.

  • Participate in care plan meetings and work closely with the interdisciplinary team.

Education & Training
  • Provide nutrition education to residents, families, and staff as needed.

  • Stay current on best practices in geriatric nutrition and implement evidence-based approaches.

Qualifications:
  • Education: Bachelor's degree in nutrition, dietetics, or a related field.

  • Licensure: Must be licensed as required by the State of New Jersey.

  • Experience: Previous experience in a long-term care, skilled nursing facility, or geriatric setting preferred.

Physical Requirements:
  • Ability to move throughout the facility and visit residents,

  • Must be able to lift up to 25 lbs. occasionally.
