Job summary

Fallsview Healthcare Center in New Jersey is seeking a Dietitian to join our dedicated team. This role involves performing comprehensive nutritional assessments on admitted patients, counseling individuals and groups on balanced nutrition, and preparing tailored meal plans that respect medical and cultural needs.

The Dietitian will provide weekly counseling, document care, and collaborate with the care team to monitor nutritional status and outcomes.

Responsibilities

• Completes a comprehensive nutritional assessment on each patient upon admission
• Counsels patients on principles of well-balanced nutrition in individual sessions and conducts group nutrition discussions
• Prepares individualized meal plans considering medical, nutritional, age, cultural, religious and ethical needs
• Provides weekly nutrition counseling for patients and families as needed
• Provides weekly updates on nutritional status as part of team rounds
• Completes required documentation and reports per policies
• Demonstrates accountability for patient information
• Monitors meals and snacks and models normal eating with disordered patients
• Communicates with referring/outpatient dietitians at admit, during treatment, and upon discharge

Dietitian Responsibilities:

• Completes a comprehensive nutritional assessment on each patient upon admission
• Counsels patients on principles of well-balanced nutrition in individual sessions and conduct group nutrition discussions
• Prepares individualized meal plans that consider the needs of the patient including; medical, nutritional, age, cultural, religious and ethical concerns
• Provides at minimum weekly individual nutritional counseling for patients and family (as needed)
• Provides updates on the nutritional status of each patient weekly as part of team rounds/meetings and as requested
• Completes all required documentation and reports in a timely manner, maintaining compliance with the policies and procedures
• Demonstrates accountability for the proper use of patients protected health information
• Monitors meals and snacks, including eating/modeling normal eating with eating disordered patients
• Communication with referring and/or outpatient dietitian upon admit to program, during course of treatment as needed, and upon discharge from program
