St. Luke Health Services is seeking a Diet Aide/Technician to support the Registered Dietitian in implementing diet orders and interviewing residents or families for food preferences.

The role includes completing low risk nutritional assessments, attending team conferences with residents and families, and contributing to care planning and quality measures under the direction of the RD.

Formación

• Associate’s degree in nutrition or dietetics is required.
• Experience working with a Registered Dietitian is required.

Responsabilidades

• Work independently under the direction of the RD to complete timely assessments.
• Follow Standards of Nutritional Care to perform low risk nutritional assessments and document in EMR using ADIME or PES formats.
• Assist the RD with in-person interviews for residents.
• Input resident information, preferences, and diet changes in PCC.
• Review menus weekly and report discrepancies to the RD.
• Collaborate with the IDT to address nutritional changes needing referrals.
• Attend morning rounds with the IDT and report episodic nutritional concerns.
• Assist with Quality of life notes and QA audits as directed by the RD.
• Identify residents at risk for malnutrition or malnutrition risk indicators for escalation.
