Job summary

Fresenius Medical Care North America is hiring an entry level Master of Social Work (MSW) to provide psychosocial services to patients at the CA137 Kearny Mesa JV clinic. The role supports in-center and home dialysis patients, collaborating with the care team to promote adjustment and quality of life.

The MSW will assess psychosocial needs, assist with care planning, counsel patients and families, and connect them with community resources.

Qualifications

• Master's in Social Work required.
• State temporary license, Registered Social Worker Clinical Intern, Associated Social Work license or Provisional LCSW may be acceptable as required by state guidelines.
• 0–2 years of related experience.

Responsibilities

• Assess patients' psychosocial status and contribute to care planning.
• Provide counseling and supportive interventions to patients and families.
• Refer to community resources and coordinate with the interdisciplinary team.
• Document MSW interactions and participate in quality improvement activities.
