Job summary

Satellite Healthcare is seeking a Medical Social Worker to educate patients with kidney disease on treatment choices, health, and wellness to enhance quality of life.

You will support patients’ social and emotional well-being, conduct psychosocial assessments, participate in care planning, and help them cope with chronic kidney disease and dialysis in a multidisciplinary team. A passion for compassionate, patient-centered care and strong communication are essential.

Qualifications

• Master's Degree in Social Work from an accredited CSWE institution.
• MSW required. LCSW/ACSW preferred, LCSW preferred (CA). Licensure/LMSW required in states with licensure (TX, TN).

Responsibilities

• Educate patients on treatment choices to improve health outcomes and quality of life.
• Conduct psychosocial assessments and participate in care planning and team reviews of psychosocial needs.
• Support the social and emotional well-being of patients and assist in coping with chronic kidney disease and dialysis.
