Qualifications

• Master's Degree in Social Work from an accredited CSWE institution.
• MSW license required; LCSW/ACSW preferred (CA). Licensure/LMSW required in states with licensure (TX, TN).
• One (1) year of work experience in a behavioral or healthcare-related setting, preferred.

Responsibilities

• Educate patients on treatment choices, health and wellness to enhance quality of life for those living with kidney disease.
• Support the social and emotional well-being of patients and assist in coping with chronic kidney disease and dialysis.
• Conduct psychosocial assessments and participate in care planning and team reviews of psychosocial needs.
