Satellite Healthcare is seeking a Medical Social Worker to educate patients living with kidney disease and support their social and emotional well-being. You will conduct psychosocial assessments, participate in care planning, and help patients cope with chronic kidney disease and dialysis treatments.

You will work with a multidisciplinary team to enhance patient quality of life, address psychosocial needs, and contribute to a compassionate patient experience.

Qualifications

• Master's degree in Social Work from an accredited CSWE institution.

Responsibilities

• Educate patients on treatment choices to enhance quality of life.
• Support the social and emotional well-being of patients.
• Conduct psychosocial assessments and participate in care planning.
• Provide management of services to help patients cope with CKD and dialysis treatments.
