Job summary

UPMC Western Maryland's Outpatient Dialysis Department seeks a full-time Social Worker for dayshift (Mon–Fri); Saturdays may be required. All MSW level licensed Social Workers are encouraged to apply.

Compensation will reflect years of experience and education within the career ladder. This role includes psychosocial assessments, crisis intervention, and discharge planning to support patient needs and families throughout the hospital stay.

Qualifications

• MSW licensed social worker required in Maryland.
• Full-time availability; dayshift Monday–Friday; Saturdays may be required.
• Experience in psychosocial assessments and discharge planning preferred.

Responsibilities

• Perform comprehensive psychosocial assessments addressing social, economic, cultural and behavioral factors.
• Educate and advocate for patients and families on medical decisions, discharge planning, and advance directives.
• Collaborate with healthcare teams to develop individualized care plans.
• Maintain professional practice and patient confidentiality per NASW and state guidelines.
• Monitor and reassess care plans every 3 days or as status changes.
• Provide counseling and crisis intervention for patients and families as needed.
• Document interventions accurately in medical records per policies.
• Mentor MSW students or field instructors when available.
• Maintain knowledge of resources and assist with post-acute care referrals.
