Dialysis Clinic, Inc. seeks a Social Work Intern to assist the Licensed Clinical Social Worker in assessing psychosocial and financial needs of peritoneal, hemodialysis, home, kidney transplant, and pre-ESRD patients and their families.

The role involves counseling on ESRD-related issues and coordinating transportation needs. DCI emphasizes a strong nonprofit mission, patient-centered care, and a lower caregiver-to-patient ratio in a comfortable clinical setting.

Qualifications

• Completion of an accredited Master Social Work Program
• Currently licensed in the state or working toward clinical licensure.

Responsibilities

• Initially assesses the patient's psycho-social needs by collecting data and applying psycho-social recommendations to develop a care plan.
• Counsels patients and families regarding ESRD therapies and financial aspects, assisting with financial forms and transportation contacts.
• Maintains professional relationships with staff and participates in administrative and staff programs as required.
• Gathers and documents data in the patient’s medical record to assess and modify the care plan per regulatory requirements.
• Develops educational materials to support patient and staff education.
• Participates in care plans and the care planning process for patients.
