Dialysis Clinic, Inc. is seeking a Social Work Intern to support ESRD patients and families in a nonprofit clinical setting.

You will work with the Licensed Clinical Social Worker to assess psycho-social and financial needs across peritoneal, hemodialysis, home, and transplant patients. This full-time role includes counseling, helping with financial forms and transportation needs, and participating in care planning, with paid training, competitive pay, and Sundays off as part of our supportive

Responsibilities

  • Assess psycho-social needs by collecting data and collaborating with the attending physician to plan optimal care.
  • Counsel patients and families on ESRD therapies and financial aspects; assist with forms and transportation contacts.
  • Maintain professional relationships with staff; participate in meetings and ongoing staff programs.
  • Document data in medical records to adjust care plans as required by regulations.
  • Develop educational materials for patients and staff.

The Dialysis Social Work Intern, works with the Licensed Clinical Social Worker to assess the psycho social and financial needs of Peritoneal patients, Hemodialysis patients, home patients, kidney transplant patients and pre-ESRD patients; counseling patients and their families on ESRD related issues including financial and transportation needs.

Schedule: Full-time, five 8-hour shifts starting at 8am

Responsibilities

What You Can Expect:

  • Initially assesses the patient's psycho-social needs by collecting and/or obtaining subjective and objective data and utilizing that information and thepsycho-social recommendations of the attending physician to develop and implement a plan for the provision of optimal care.
  • Counsels patient and families, separately and together, regarding ESRD therapies and the financial aspects of ESRD modalities, assisting in the completion of financial forms, and assisting in transportation by supplying information and making contacts with/for the patient.
  • Maintains professional working relationship with other staff of DCI. Participates in administrative and staff programs as required. This includes attending staff meetings and in service meetings.
  • Gathers and documents in the patient's medical record data to assess and modify the care plan in accordance with regulatory requirements on an on-going basis.
  • Compiles and develops educational materials to be used as teaching aids in education of patients and staff.
  • Participates in the care plans and the care planning process for patients.

Education/Training:

  • Completion of an accredited Master Social Work Program
  • Currently licensed in the state or working toward clinical licensure.
