Dialyze Direct is seeking a Travel RN in Illinois with dialysis experience to provide high-quality nursing care and home support as needed. The role involves 75% travel, a full-time fixed schedule, and coordination with nephrologists and SNF partners.

Requirements include a valid IL RN license, 6 months RN experience preferred, 3 months hemodialysis experience, BLS certification, and the ability to lift up to 35 pounds. This is a safety-sensitive position offering comprehensive benefits.

Qualifications

• Valid RN license in IL.
• 6 months RN experience preferred.
• 3 months of hemodialysis experience.
• In the state of Texas, 6 months of hemodialysis experience within the last 24 months.

Responsabilités

• Must be able to function at a high level with little oversight to provide nursing care, training, and home support (as needed).
• Works collaboratively with Dialyze Direct team members, skilled nursing home facility representatives and dedicated Nephrologists.
• Collects and records relevant patient health data in the course of/pre hemodialysis treatment.
• Develops and maintains a plan of care that describes interventions to attain expected outcomes.
• Implements and records interventions to attain expected outcomes in EMR.
• Performs laboratory collection, processing, and shipping procedures and reschedules missed or inadequate laboratory collections.
• Perform clerical tasks in addition to patient care (i.e., filing, inventory management, admission documentation, creating patient charts, etc.).
• Maintains a positive and respectful attitude to all patients and colleagues.
• Occasional travel to other nearby locations; and
• Medication administration and documentation.
