Job summary

Montefiore Medical Center is seeking an experienced Registered Nurse (RN) to uphold nursing standards and lead coordination of patient care. The RN will implement interdisciplinary plans, mentor staff, and ensure comprehensive care documentation in line with policy.

Responsibilities include leadership in patient care delivery, creating and monitoring care plans, and ensuring high-quality outcomes for nephrology/dialysis patients. Strong English communication is required.

Qualifications

• New York State License for Registered Professional Nurse and current registration or valid permit in NY as an RN.
• Bachelor’s Degree (BSN) preferred.
• Associate Degree (ASN) required.
• Basic Life Support (BLS) Training.
• Infection Control Training.
• Certification in specialty preferred.
• Nephrology/Dialysis experience.
• 1 year of experience preferred.
• Effective English communication skills, both written and oral.
• Ability to use basic data entry and retrieval systems.
• Use of patient care equipment.
• Use of automated clinical information systems.

Responsibilities

• Demonstrates leadership in the coordination and delivery of patient care.
• Coordinates an interdisciplinary plan of care.
• Assures 100% of assigned patients have a documented plan of care.
• Ensures completion of patient care assignments 100% of the time.
• Interdisciplinary plan of care aligns with LOS parameters for all patients.
• Delegates care tasks to team members within capabilities.
• Ensures delegated tasks meet established standards of practice.
• Escalates issues per MMC policy and procedure.
• Establishes a therapeutic relationship with patient/family.
• Assumes charge nurse assignment.
• Mentors/precepts other associates.
