Job summary

Rochester Regional Health's Bay Creek Dialysis center in Webster, NY is seeking a Registered Nurse. The role supports patients with chronic kidney disease and requires rotating shifts during site hours 5a-9:30p.

You will perform assessments, develop care plans, collaborate with the interdisciplinary team, educate patients and families, and participate in performance improvement activities to ensure high-quality care.

Qualifications

• Diploma or Associate’s Degree in Nursing required; Bachelor’s Degree in Nursing preferred.
• Registered Nurse license in New York State.
• Basic Life Support (BLS) certification.

Responsibilities

• Performs initial and ongoing assessment of the patient and documents findings.
• Develops and documents plan of care and collaborates with the interdisciplinary team.
• Implements care and education plans, maintaining safety and appropriate nurse-patient relationships.
• Evaluates patient responses and revises plans of care as needed.
• Provides education to patient and family using teach-back methods.
