Job summary

Rochester Regional Health is seeking a Licensed Practical Nurse in Hemodialysis at the Webster-area Bay Creek dialysis location. The role supports the RN, conducts assessments, administers dialysis, and participates in treatment plans.

The position offers a 40-hour workweek with days, evenings and weekends, and requires NY LPN license, BLS, and dialysis-specific training plus ongoing education opportunities.

Qualifications

• Completion of NYS LPN training for outpatient hemodialysis units.
• Current NYS LPN license and BLS certification.
• Annual competency for accessing central lines in HD unit.

Responsibilities

• Conducts and documents a thorough assessment of each patient’s medical status upon admission and throughout the patient’s course of treatment.
• Assists in the implementation of the patient’s treatment plan under the direction of the RN.
• Complies with department guidelines for evaluation, treatment and documentation.
• Maintains standards and policies in line with hospital and regulatory requirements.
• Maintains clinical currency through continuing education opportunities.
• Administers IVP medications specific to dialysis patients.
• Initiates and discontinues dialysis with AV fistulas, AV grafts and CVCs.
• Cleans and redresses CVC skin sites.
• Draws blood from CVC sites.
• Responds to CVC line separations.
