Job description

Centers Dialysis Care is seeking Registered Dialysis Nurses (RN) for our home hemodialysis program in a skilled nursing facility located in Bronx, NY.

Duties

1. Assess patient status and implement care plans in collaboration with the interdisciplinary team.
2. Coordinate care and educate patients.
3. Administer medications and treatments as directed by physicians.
4. Monitor and document patient responses to treatments.
5. Mobilize resources for complex cases to maximize patient participation in recovery.
6. Integrate assessment data to anticipate healthcare needs.
7. Ensure effective handoff communication.
8. Adjust staffing patterns as needed based on unit needs.
9. Provide individualized comfort measures using evidence-based practices.
10. Participate in performance improvement activities.
11. Perform other related duties as required.

Requirements

• Active NY RN License
• Dialysis experience required
• Nx Stage machine experience is a plus
• CPR Certification

Rate: $45.00 - $55.00 per hour
