Qualifications

• High School diploma or G.E.D. is required.
• CMS-approved state or national certification within the required state or CMS timeline.
• All state licensure, education, and training required.
• CPR certification required.
• Previous patient care experience in a hospital setting or related facility preferred.

Responsibilities

• Assist in providing patient-specific education on adequacy measures (OLC, AMP, UKM) and disease process/access.
• Welcome patients and check wellbeing since last treatment.
• Evaluate vascular access for patency and cannulation; administer heparin as delegated.
• Record vitals, weight, and vascular access evaluation pre-treatment.
• Initiate dialysis per orders (QB, QD) and monitor response to therapy.
• Document treatment data and complete Hemodialysis Treatment Sheet.

Education
