Job summary

Fresenius Medical Care is seeking a dialysis patient care team member to support the safe operation of hemodialysis machines and deliver care under licensed nurse supervision. The role includes monitoring patients, initiating treatment, and recording vital signs and treatment data in accordance with policy.

Responsibilities also cover vascular access evaluation, post-treatment discharge procedures, infection control, and participation in CQI activities to improve patient outcomes.

Qualifications

• High School diploma or G.E.D. required.
• CMS-approved state and/or national certification requirements within the required state or CMS timeline.
• All appropriate state licensure, education, and training (if any) required.

Responsibilities

• Assist in you handling patient education on adequacy measures including Online Clearance Monitoring (OLC) and Urea Kinetic Modeling (UKM).
• Welcome patients and check their wellbeing after the last treatment.
• Evaluate vascular access for patency and perform cannulation as delegated.
• Initiate dialysis treatment according to prescribed orders (QB and QD).
• Monitor patient response to dialysis therapy and document vital signs and treatment parameters.
• Ensure safety alarms are enabled and record data in the Hemodialysis Treatment Sheet.

Education
