Concerto Renal Services is actively seeking a Dialysis Nurse to join its team. The nurse will supervise dialysis care, work hands-on with the team, and ensure high-quality patient care in a sub-acute dialysis setting within a skilled nursing facility or long-term care facility.

The role emphasizes direct patient care, coordination with physicians, and documentation to maintain safety and regulatory standards while promoting renal health education for patients and families.

Qualifications

• Provides direct hemodialysis care, assesses patients, administers medications, and educates patients on renal health.
• Supervises dialysis technicians and ensures compliance with standards.
• Monitors patient response and documents interventions.
• Coordinates with physicians and healthcare team to ensure continuity of treatment.
• Maintains electronic medical records and adheres to safety protocols.

Responsabilités

• Provides direct hemodialysis care, performs assessments, administers medications, and educates patients on renal health.
• Delegates tasks to dialysis technicians, supervises their performance, and ensures compliance with clinical standards and regulations.
• Monitors patient response to dialysis, adjusts treatment plans as needed, and ensures accurate documentation of interventions and outcomes.
• Works closely with physicians and healthcare team members to coordinate care, report critical lab results, and ensure continuity of treatment.
• Maintains accurate electronic medical records, ensures timely completion of care plans, and adheres to safety and regulatory protocols.
