Responsibilities

• Conducts and documents a thorough assessment of each patient’s medical status upon admission and throughout the patient’s course of treatment.

• Complies with department guidelines for evaluation, treatment and documentation.
• Maintains standards and policies in line with hospital and regulatory requirements.
• Maintains clinical currency through continuing education opportunities.
• Administers IVP medications specific to dialysis patients.
• Initiates and discontinues dialysis with AV fistulas, AV grafts and CVCs.
• Cleans and redresses CVC skin sites.
• Draws blood from CVC sites.
• Responds to CVC line separations.

SUMMARY:

As a Licensed Practical Nurse in Hemodialysis, your specialized skill set and ability to collaborate with the Registered Nurse is vital to exceptional patient care. Rochester Regional Health offers a full range of Dialysis treatment options to patients with chronic kidney disease. We treat adults from people from multiple backgrounds, cultures and ethnicities. We care for patients temporarily, either their kidney function recovers or they return to their home dialysis center, move or get a transplant; or some are with us on an ongoing basis. Patients have dialysis treatment anywhere from 2-3 days a week, from 3 to 5 hours each treatment. Our staffing model consists of RN’s, LPN’s and Techs.

RESPONSIBILITIES:

• Conducts and documents a thorough assessment of each patient’s medical status upon admission and throughout the patient’s course of treatment.
• Assists in the implementation of the patient’s treatment plan under the direction of the RN.
• Complies with the evaluation, treatment, and documentation guidelines of the department.
• Acts in compliance with hospital, regulatory, and professional standards and policies.
• Maintains clinical currency by pursuing continuing education opportunities.
• Administers IVP medications specific to dialysis patient population.
• Initiate and discontinue dialysis treatments using multiple access sites including AV fistulas, AV grafts and Central Venous Catheters (CVC).
• Clean and redress CVC skin sites.
• Draw Blood from CVC.
• Respond to CVC line separations.

REQUIRED QUALIFICATIONS:

• Graduate from an accredited School of Practical Nursing OR ASN/BSN in Nursing.
• Current New York State Licensed Practical Nurse license.
• Completion of the NYS Department of Health and NYS DOE approved training curriculum for LPN’s working in Outpatient Hemodialysis Units (completed as part of unit based orientation).
• Completion of the NYS DOH and NYS DOE approved annual competency for LPN’s accessing central lines in a hemodialysis unit (completed as part of unit based orientation).
• Basic Life Support (BLS) Certification required.

PREFERRED QUALIFICATIONS:

• One year experience preferred.
• At least 6 months LPN experience preferred

PHYSICAL REQUIREMENTS:

Moderate - Clinical - Moderate clinical roles are patient facing roles that require continual movement, including standing, walking, and lifting 20–50 lbs., along with pushing and pulling medical equipment and assisting with patient positioning. Employees perform frequent bending, twisting, and reaching while completing fine motor tasks in busy or confined clinical environments. High sensory acuity is essential for monitoring patient status and responding to alarms or safety concerns. Emotional and cognitive demands are substantial, involving decision-making under pressure, responding to emergencies, and maintaining sustained focus. Environmental exposures include infectious agents, bodily fluids, chemicals, and radiation. TB testing, mask-fit testing, and physical exams are common requirements. PAY RANGE:

$27.80 - $33.92 CITY: Webster POSTAL CODE: 14580 The listed base pay range is a good faith representation of current potential base pay for a successful full time applicant. It may be modified in the future and eligible for additional pay components. Pay is determined by factors including experience, relevant qualifications, specialty, internal equity, location, and contracts.
